Azalea Kim MD, MBA, MPA, is a primary care physician and Medical Director for Applied Health Data Science and Strategy for Duke Forge, Duke University's center for health data science, and Duke Health Technology Solutions. She drives collaboration across Duke to develop and execute strategy for analytics in high value care and to support performance in value-based contracts. She oversees development and application of data science models to ensure alignment with high value care, and provides clinical leadership on development of models related to advance care planning and social determinants of health.
Azalea is a practicing primary care physician who co-directs innovation at Duke Outpatient Clinic, including data-driven design and implementation of advanced primary care models for complex patients with a focus on serious illness planning, behavioral health, and addressing social drivers of health. Azalea has prior management and innovation experience in accountable care organizations and population health management operations.
Azalea received her MD from the Icahn School of Medicine at Mount Sinai, MBA from Harvard Business School, MPA from the George Washington University, and training in Internal Medicine at Duke University Health System.
